What Is Colorado Visiting Nurse Association?

The Visiting Nurse Association (VNA) provides the complete array of rehabilitation services to ensure patients have access to the very specific, intense therapies that help restore patients to their former functional status after an illness. VNA offers:Skilled analysis of functional capacities of patients; e.g., muscle strength, sensation, pain, neuromuscular status, coordination, circulation, pulmonary capacity, cognition, emotional response, psychosocial functioning. Proficiency in developing plan of care for treatments and therapeutic interventions and in setting goals with patient and family on personalized basis. Competency in assessing need for other disciplines and supportive services and in evaluating family support systems and potential for follow through care. Partners with physician and other disciplines on treatment plan and modalities and in resolving patient problems related to safety, mobility and independence. Skill in direct care and in patient/family instruction on home treatment program and equipment; e.g., adaptive, safety, supportive devices for extremities. Expertise in identifying factors in home environment that must be modified so patient can safely stay at home and in teaching family how to deal with possible emergencies. Specialists in working with patients from geriatric to pediatric; e.g., working with developmentally delayed children to increase/decrease muscle tone. VNA professionals include: Physical therapists, who focus on gross functional activities, motion/mobility, balance, posture and gait, so patient becomes more independent Occupational therapists, who focus on activities of daily living, improving cognition, perceptual-motor, upper extremity function Speech pathologists, who focus on verbal/nonverbal communication, cognition, oral motor activity, voice/speech/linguistic disorders and dysphagia.
