Home Health Care: Our goal is to keep our patients at home, improve quality of life, and assist our patients to recover and regain independence through the delivery of clinical excellence and compassionate care. Services include; skilled nursing, physical, occupational and speech therapy, palliative care, medical social workers, specialists in wound care, ostomy, and IV/INR. We also provide personal home care aides, 24-hour phone support, Alzheimer and Dementia support and medication management. We offer TeleHealth technology to monitor, support and promote the clinical health of our patients, at no cost. Hospice Care: The goal of hospice care is to enhance the quality of life of a patient living with life limiting illness through comfort care and pain management. Our services include physician care, nursing care, home health care aides, medical social workers, spiritual and bereavement counselling , interfaith chaplains, hospice trained volunteers and 24 hour phone support. Pet Peace of Mind Program: Keeping our patients and their pets together through Hospice Care. We are a proud program affiliate, of the national Pet Peace of Mind Program. For many in hospice care, a decline in physical condition impacts a patient's ability to enjoy socializing and maintain human relationships over time. Pets fill that relational void and they offer unconditional love, acceptance comfort and companionship at at time when it's needed most. The program enables hospice patients to keep their pets at home throughout their end of life journey. The program provides volunteers, food, veterinary care and other pet care needs at no cost to patients. We also assist with re-homing a patient's pet, should the need arise.
