A partner of CommuniCare Health Services, which is a leader in post-acute care, CommuniCare Advantage (CCA) provides accessible and affordable health insurance for special needs populations as defined by CMS. CCA offers two plans: • An Institutional Special Needs Plan (ISNP) for qualifying residents of participating nursing facilities • A Chronic Condition Special Needs Plan (CSNP) for eligible individuals with diabetes mellitus, chronic heart failure, and/or cardiovascular disease These plans design services and benefits around the unique health needs of these special populations.
